We have assisted patients, caregivers and healthcare providers in 43 countries around the world! We look forward to assisting you! In order to serve your needs, we ask that you take a few minutes to complete this form, to the best of your ability, and return it to us with your subscription membership dues. We respect and follow HIPAA guidelines to insure your privacy. The information you provide to us, including names, email addresses and phone numbers, is shared only with other members who have agreed to respect our privacy policy and who share your same diagnosis and/or other information that you request be exchanged with others. We do not utilize "chat rooms" or internet "message boards". We do not share your information with healthcare providers or cancer societies. Our subscribing members communicate and share resources directly via email, telephone calls and personal visits. We are real people, just like you! We are an "all volunteer" networking organization. We do not receive compensation from healthcare providers, cancer societies or government agencies. Your subscription membership dues are utilized to help defray some of the costs of producing and maintaining our WEBSITE at www.pmppals.org, publishing our NEWSLETTER, distributing our monthly email BULLETIN, responding to your telephone calls, emails and written correspondence (usually within 12-24 hours of receipt), production and distribution of the Pals' HANDBOOK, office supplies, computer hardware equipment, etc. Please complete and return this form to us, via postal mail, with your subscription membership dues: PMP PALS Application 08 (click the link to open the printable form)
