Abstract

 Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment option for selected patients with pseudomyxoma peritonei (PMP) and diffuse malignant peritoneal mesothelioma (DMPM).

Tumor infiltration of the hemidiaphragm requiring partial resection occurs as a result of large volume and/or invasive disease at this anatomic site. Transmission of disease from abdomen to chest is a great danger in this group of patients.

From a prospective database, patients who had diaphragm resection and then hyperthermic thoracoabdominal chemotherapy (HITAC) as a component of a cytoreductive surgical procedure were identified.

 Data from control patients receiving HIPEC or hyperthermic intrathoracic chemotherapy (HITOC) were analyzed for comparison. The morbidity, mortality, survival, and recurrence rate within the thoracic space were presented.

Thirty patients had partial resection of a hemidiaphragm as part of a cytoreductive surgical procedure that utilized HITAC. The pharmacologic benefit of intracavitary chemotherapy administration was documented with an area under the curve ratio of intracavitary concentration times time to plasma concentration times time of 27 ± 10 for mitomycin C and 75 ± 26 for doxorubicin. Comparing percent chemotherapy absorbed for a ninety-minute treatment showed the largest for HIPEC, then for HITAC, and lowest for HITOC. The incidence of grade 3 and 4 adverse events was 43%. There was no mortality.

Adjustments in the chemotherapy dose are not necessary with HITAC. The morbidity was high, the survival was acceptable, and intrathoracic recurrence was low.

Paul Sugarbaker, MD is a professor, investigator, and lectures throughout North America and  internationally, earning numerous awards and citations. He is well known for his expertise in gastrointestinal oncology and colorectal cancer surgery and in rare diseases including pseudomyxoma peritonei, peritoneal mesothelioma, abdominal sarcomas, and peritoneal carcinomatosis.



Dr. Sugarbaker has pioneered specialty treatment procedures including cytoreductive surgery (CRS) with heated intraoperative intraperitoneal chemotherapy (HIPEC) and hyperthermia treatments using the Thermotron RF-8.
Dr Sugarbaker  has published more than 780 scientific articles in peer-reviewed journals, numerous book chapters and textbooks. His textbook about sarcoma surgery has been translated into five languages.

Dr Paul H Sugarbaker hosted a CRS and HIPEC workshop at the Washington Hospital Center,on June 20 & 21, 2012.  

Prior to the symposium, Dr Sugarbaker told PMP Pals,

"We will perform a live surgery including a full day of didactic teaching by world experts.

Dr Sugarbaker said, "The emphasis of this year's workshop is on 'proactive management.' If we can get patients with appendix malignancy, peritoneal mesothelioma, and colorectal cancer treated early with a small volume of disease, the long term results are superior and the quality of life issue less prominent. 

Of course, if these treatments become more accepted for colorectal cancer patients with peritoneal metastases we are expecting approximately 15,000 new patients per year in the United States.  Certainly, that will require many additional centers of excellence for CRS plus HIPEC."

Referring to PMP in particular, Dr H Sugarbaker tells PMP Pals, "Now there are are many 10-year and 20-year survivors of this rare disease.  Definitely, a new chapter in the medical history of pseudomyxoma peritonei has been written."
